Broken or Cracked Tooth: What to Do in the First Hours and Treatment Options

Teeth can break or crack when biting something hard, in a fall or during sport. Even a small chipped corner can grow over time, and a larger fracture may expose the nerve. The right first steps and seeing a dentist promptly significantly increase the chance of saving the tooth.

Why Do Teeth Break or Crack?

  • Falls, accidents and contact sports
  • Biting hard objects such as ice, seeds or bones
  • Clenching and grinding
  • Teeth weakened by large fillings or root canal treatment
  • Large, untreated cavities
  • Sudden temperature changes (an iced drink straight after very hot food)

Types of Fractures and Cracks

  • Craze lines: fine surface lines in the enamel, usually painless and often needing no treatment.
  • Chipped corners: a piece of tooth breaks off; if the nerve is not involved, it can usually be repaired with a filling.
  • Fractures involving the nerve: a pink/red dot may be visible on the broken surface, with marked pain and sensitivity.
  • Cracked tooth: the crack may be invisible; sharp pain when biting, especially on release, is typical.
  • Vertical root fractures: start in the root and are harder to diagnose and treat.

First Steps When a Tooth Breaks

  1. Rinse gently with warm water.
  2. If it bleeds, press lightly with clean gauze for about 10 minutes.
  3. For swelling, apply ice wrapped in a cloth to the outside of the cheek.
  4. Keep the broken fragment if you can find it, in milk or saline; in some cases it can be bonded back.
  5. If a sharp edge cuts your tongue or cheek, cover it temporarily with sugar-free gum or dental wax from a pharmacy.
  6. Avoid chewing on that side and avoid very hot or very cold food.
  7. See a dentist as soon as possible.

If a Tooth Is Knocked Out: The First 60 Minutes Matter

If a permanent tooth is knocked completely out of its socket, time is critical:

  • Hold the tooth by the crown (white part), not the root.
  • If it is dirty, rinse it for a few seconds with milk or saline; do not scrub or wipe it.
  • If possible, put it straight back into the socket and bite gently on a clean cloth to hold it.
  • If you cannot, carry it in milk or saline, or inside your cheek. Do not store it in tap water or a dry tissue.
  • Get to a dentist without delay, ideally within the first hour.

Knocked-out baby teeth are not put back, as this can damage the permanent tooth underneath, but the child should still be examined.

Treatment Options

  • Composite filling and bonding: small and medium fractures are repaired with tooth-coloured composite, with good aesthetic results on front teeth.
  • Re-attaching the fragment: possible in some cases if the piece was stored correctly.
  • Veneers: for larger fractures on front teeth, veneers may be considered for aesthetics and strength.
  • Root canal treatment: if the nerve is involved, root canal treatment, usually followed by a crown, can save the tooth.
  • Crown: a dental crown may be recommended to stop a crack from spreading in a weakened tooth.
  • Extraction and implant: if the fracture runs down the root and the tooth cannot be saved, it is extracted and the gap can be restored with a dental implant or bridge.

Preventing Broken Teeth

  • Wear a custom mouth guard for contact sports.
  • Do not bite ice, pens or nut shells, and do not use your teeth to open packaging.
  • If you clench, ask your dentist about a night guard.
  • Ask at check-ups whether heavily filled or root-treated teeth should be protected with a crown.

Can a broken tooth heal on its own?

No. Tooth tissue cannot regenerate. Even small fractures can grow or become prone to decay, so an examination is recommended.

Should I see a dentist if the broken tooth does not hurt?

Yes. No pain may mean the fracture has not reached the nerve, which is usually when repair is simplest. Waiting can make treatment more complex.

How is a cracked tooth diagnosed?

Cracks are not always visible to the eye or on X-rays. Bite tests, examination with special light and an assessment of symptoms are used.

Can a broken front tooth be repaired the same day?

Small and medium fractures that do not involve the nerve can often be repaired with composite in a single visit. Larger fractures may need several appointments.
